Instructions for Sponsorship by Mail or Fax
To donate by mail or fax go to www.cantoo.org.au

1.	Click on Sponsor a Participant
2.	On the menu bar click on Download a Sponsorship Form

Sponsorship forms should be sent to:

After a sponsorship has been received by mail or fax your sponsor will be posted a tax deductible receipt, and you will receive an email notification of this sponsorship.

Cheques
Cheques should be made out to Cure Cancer Australia Foundation

Send the cheque and a completed sponsorship form to:

Cash Donations
If you have a donation in cash you need to complete a sponsorship form and hand this form and cash into your team captain at training on Wednesday. We will not be able to take cash (or cheques) at training on Saturday.

An official receipt will be issued by Cure Cancer Australia.
